David (Dave) Lawrence Hazelwood, Jr, 76, passed away peacefully with his wife by his side at his home August 10, 2021.   Born in Mayfield, Ky December 20, 1944 to David Lawrence Hazelwood, Sr and Mary Nell Thomas Hazelwood.  In addition to his parents, he is preceded in death by his brother, Donnie Hazelwood.  Also surviving along with his wife of twenty-one years, Stephanie Robertson Hazelwood is his sister, Diana Hazelwood Owens (Dale) , his nephew, Dale Owens, Jr. and Roxy, his canine companion;
step-daughter, Jessica Puckett, step-sons Joey Puckett and Justin Puckett.  Dave served in the United States Marine Corps, undercover narcotics office in Texas for many years, was a chief deputy of Henry County Sheriff's Department, and for a number of years before retirement was a life and health insurance agent/manager.   Lifetime member of VFW, Member of Tennessee Valley Community Church, Paris, TN.  A Celebration of Dave's life will be in Paris, TN.
